An increase in perianal ulcers related to the use of hemorrhoidal ointments has been observed during the COVID-19 pandemic. The case of an 82-year-old man with a significant perianal ulcer attributed to the use of a hemorrhoidal rectal ointment is presented. After ruling out other causes through exclusion criteria and biopsy, complete healing of the ulcer was achieved through a multidisciplinary intervention and therapy with autologous growth factors. This case has been sparsely reported in the literature, despite the hemorrhoidal ointment being on the market for over 40 years. Medical evaluation before prescription and patient follow-up are recommended.
