Combination of advanced therapies to cure a Martorell Ulcer

Abstract

Atypical ulcers are included within the group of complex wounds, among which the Martorell hypertensive ulcer stands out. These skin wounds derive from ischemic processes caused by the obstruction of arterioles that affect elderly patients with a long history of hypertension.

We present the case of an 82-year-old woman who was independent for basic and instrumental activities of daily living, without cognitive impairment, with initial frailty, and with a Martorell ulcer.

The wound resolved after 14 months, with the participation of various levels of care, one of which was the clinical wound unit, where 8 sessions with PRP and 3 sessions of punch skin grafting were carried out in the last 5 months. It should be noted that in both therapies, the patient showed a significant change in the pain of the ulcer, going from a VAS of 7 to a VAS of 4 with the PRP and even reducing it to zero with the micrografts in seal, facilitating the progressive withdrawal of the opioids.
