The TR2Lab Ph.D. student, Anna Torné, presented a poster at the COHEHRE Conference 2019 which took place from the 3rd to the 5th of April in Vic. In the poster “Patients Attended by the Interdisciplinary Geriatric Assessment Team (IGAT) and Its Trajectory” (1) Anna Torné showed some of the results obtained in her Ph.D. Thesis. Mainly, Comprehensive Geriatric Assessment by an interdisciplinary team identifies hospitalized patients with more complex needs outside hospital geriatric areas. This identification of patients with higher functional loss, higher frailty, and more geriatric syndromes aids to know who will benefit from an interdisciplinary approach (physiotherapists, doctors, nurses, therapists, psychologists, speech therapists, social workers, and pharmacists) like the one offered by Intermediate Care Center at destiny.
Congratulations Anna for such excellent achievement!
