“Creation, Training, and Implementation of an Integrated Community Health Care Approach Based on Engagement in Meaningful Activities” (HABIT Project) is an Erasmus+ KA220-VET Cooperation Partnership (VET) funded by the European Union. The project develops and tests the IMPACT Approach, an integrated, community-based approach of care that places meaningful activities at the centre of health and social interventions for people experiencing social isolation, chronic conditions, and/or community disconnection.
Coordinated by the University of Vic – Central University of Catalonia (UVic-UCC, Spain), HABIT brings together eight partners from Spain, Belgium and Romania, including higher education institutions, VET centres and community/social care organisations. Together, they co-create training programmes for Community Care Facilitators, community and health agents, and VET students in health and social care, fostering interdisciplinary teamwork between health and social services at community level.From November 2025 to November 2028, the consortium will: (1) co-design and validate the IMPACT Approach and its training curriculum; (2) develop and deliver blended training and e-learning modules; (3) map community resources and services to support social prescribing and meaningful activity-based interventions; and (4) carry out real-life community pilots. HABIT will create a HABIT Knowledge Hub with open-access training materials, tools and guidelines, as well as policy recommendations to support the long-term scalability and sustainability of integrated, community-based care models across Europe.
