MEETING. Fina Clapera, member of the research line in Innovation of wound healing at TR2Lab, represented the Clinical Wound Unit of the Hospital Universitari de la Santa Creu de Vic in the Workshop of Multidisciplinari Care. Professional Experiences at the Acadèmia de Ciències Mèdiques i de la Salut de Catalunya i de les Illes Balears, in Barcelona on January 19th.

Fina Clapera presented the results of a pilot study to promoting the integration of different primary care teams with specialized care led by nursing through shared care and co-responsibility in the care of these patients. The project involved a clinical wound unit of a sociosanitary hospital and several surrounding basic areas.

The results of this experience brought more proximity to the patient’s field, guaranteeing the continuity of care as well as being more resolute.
